Certificates of Insurance

If you have a need to provide a certificate of insurance from the University to a third party:

1. Fill-out the certificate of insurance request form and submit one week prior to event.
2. Please upload the supporting documents (completed contract, certificate holder request form, literature of event, copy of permit application).
3.  If you are a non-Syracuse University affiliate and would like to request a certificate of insurance, or have questions, please contact: riskadmin@syr.edu, or 315.443.4011.
