Workers’ Compensation Claims

The Syracuse University workers’ compensation program provides payments for medical treatment and wages lost as a result of a work-related illness or injury.

File a Claim

If you experience a work-related illness or work injury:

  1. Notify your supervisor
  2. To file a claim for benefits, complete the Workers’ Compensation Accident Report
    1. Employee completes their section and signs
    2. Supervisor completes their section and signs
  3. Send the injury report to Risk Management by email or fax (315.443.1154)
